Is your patient approaching a transitional phase (i.e. elementary to secondary school, going away to college,  retraining etc.) where they may need additional support?

We are a consulting practice and do not replace your role as the supporting family physician. Through ongoing communication and mutual support, we strive to work alongside you in order to complement and support your patient’s health and learning progress.

Dr. Gray is an identified psychotherapist with the Ministry of Health; therefore, services provided to enrolled patients from FHN, FHO or GHC will not contribute to outside use and will not impact Access Bonuses.

Next Steps

1. Please fill out and submit a Patient Referral Form.
   
   
2. Springboard will contact the patient to schedule an initial consultation, after which you will receive a confirmation letter.
   
   
3. Approximately 3 weeks after the initial consultation, you will receive an individualized report, including any prescriptions, accommodations and recommendations we have given to your patient.
